Job Description:

A company is looking for a Care Concierge Associate to assist benefit recipients in navigating their product platform and accessing services.

Key Responsibilities

Assist members with assessments and recommend service plans based on their needs
Coordinate and schedule services while making follow-up calls to members
Help members access various community resources such as homecare and transportation

Required Qualifications and Education

High School Diploma
Experience in Public/Community Health or a related field preferred
Proven experience answering inbound calls
Experience in Social Services, Care Coordination, or Healthcare Navigation
Computer proficiency and knowledge of clerical functions
