Job Description:
A company is looking for a Connected Care Assistant to provide
member support in managing health conditions.
Key Responsibilities
Coordinate patient-care services and support members with chronic
health conditions
Engage at-risk members through calls and outreach efforts
Complete administrative tasks including managing referrals and
obtaining medical records
Required Qualifications
Prior experience in case management or healthcare setting
preferred
Ability to multitask and adapt in a changing environment
Proficient in computer technology and quick to learn new
platforms
Knowledge of community healthcare and social welfare resources
preferred
Must comply with HIPAA guidelines and maintain patient
confidentiality
